Communities Actively Living Independent and Free
Partners\’ Multipurpose Senior Service Program provides services to help seniors stay home including transportation to appointments, access to wheelchairs and ramps, installation of grab bars, emergency response services and home fall prevention services.
Eligibility Requirements
Must be able to be served within MSSP’s cost limitations, be appropriate for care management service, be currently eligible for MediCal, be certified or certifiable for placement in a nursing facility.
